Patient Treatment Forms

If you have attempted a honeybee rescue at one of our mobile clinics, through the use of an emergency rescue kit, or just on your own, we would absolutely LOVE to hear about your firsthand experiences with inter-species care! We welcome you to add your own bee stories to our community narrative. It's easy! Just follow these steps:

1. Download the patient treatment form below (right click + save the image to your computer).

2. Fill it out as best you can. This can be digital, or if you prefer to print the form and hand write your responses, that's great too.

3. Photograph, scan, or save your responses.

4. Send a copy to communitybeeclinic@gmail.com, and we'll post it online!
